Welcome to Soul Thrive Support Services. By using our services, you agree to the terms below. Please read them carefully.

Soul Thrive Support Services is a renowned NDIS provider offering support to NDIS participants. These terms apply whenever you use our services or our website.

We provide support in line with your NDIS plan and agreed service agreement. Services may include transport assistance, community access support, daily living support, and support coordination, as outlined in your individual agreement with us.

Our services are available to NDIS participants with an active NDIS plan. You must provide accurate information about your plan and funding when booking a service.

Before support begins, we will agree on the details of your service, including what is provided, how often, and the cost. Any changes to this agreement must be discussed and agreed by both parties.

Bookings should be made with reasonable notice where possible. If you need to cancel or reschedule, please let us know as early as you can. Late cancellations may be subject to NDIS cancellation guidelines.

Fees for our services follow the current NDIS Price Guide. Payment is processed according to your funding arrangement, whether self-managed, plan-managed, or NDIA-managed.

You have the right to be treated with respect and dignity at all times. You have the right to choose and control the supports you receive. You have the right to raise concerns or make a complaint without fear of losing your services.

Please provide accurate and up-to-date information about your needs and plan. Please treat our staff with respect. Please let us know as soon as possible if your circumstances or needs change.

We collect and store your personal information to provide our services safely and effectively. Your information is kept private and is only shared where required by law or with your consent.

If you are unhappy with any part of our service, please contact us directly so we can resolve the issue. You may also contact the NDIS Commission if you feel your concern has not been addressed.

Either you or Soul Thrive Support Services may end a service agreement. We ask for reasonable notice so the transition is smooth and safe.

We may update these terms from time to time. Any changes will be reflected on this page with an updated date.
